DESCRIPTION:
Since May 1, 1998, the Division of Registration has required that prospective tenants be informed at first personal meeting whether they will pay a fee, the amount of the fee and whether they will be responsible for paying a fee, regardless of whether a tenancy is created. This form satisfies the regulation informing the tenant that they will be obligated to pay a fee when a tenancy is created or lease is signed.
